When You Go Home

There are many services available for you to access to obtain help and support for yourself, family and carers.

24 Hour Helpline

The Hospice has a 24 hour helpline which is situated in the Inpatient Unit and answered by trained nurses.  Staff can give you advice and help regarding any concerns you have and refer you to an appropriate person if needed.  The number for the helpline is 01429 855558 and is available 24 hours a day.

NHS Advice

NHS Advice is a service available to you to gain advice about health needs, medication and concerns you have.  This is also available 24 hours a day if you require urgent medical help on 111.

District Nursing Team

The District Nursing Team is available for you to access, the number is 01429 522500, for Hartlepool & Tees Valley or if you live in the East Durham area the number is 0191 5692883 and out of hours, the number is 07917 517852 (East Durham).

Macmillan Cancer Support

If you have a Macmillan Nurse you can access them 9.00 am – 5.00 pm, Monday – Friday on 01429 522154 or 01429 522697.

If you live in the East Durham area the number is 0191 5692875.  For those living in the Tees Valley the number is 01642 383041.

HBC & DCC

Social Services

Social Services can also provide advice and support regarding any worries you have about your social care, benefits advice and help for you and your family.

They can be contacted on 01429 266522 for Hartlepool, 0191 5692894 for East Durham and 01642 522764 for the Tees Valley.
